چکیده

If human atrial flutter is due to re-entrant excitation, depolarization as well as repolarization must continue throughout the entire atrial cycle. It follows that the atrial vulnerable period for inducing atrial fibrillation is also continuous rather than discrete. This hypothesis was examined during cardioversion of I33 patients with atrial flutter who received 280 low-energy shocks. A composite analysis of these patients demonstrated that all intervals of the flutter cycle were equally susceptible to shock-induced atrial fibrillation. The optimal energy was found to be io Wsec. The development of atrialfibrillation was independent of the state of digitalization and was not prevented by pretreatment with atropine or propranolol. These findings are consistent with re-entry as the basic mechanism of human atrial flutter.

Safety and efficacy of ibutilide in cardioversion of atrial flutter and fibrillation.

This article reviews the safety and efficacy of ibutilide for use in patients with atrial fibrillation and flutter. Ibutilide, a class III antiarrhythmic agent, is primarily used for conversion of atrial flutter and fibrillation and is a good alternative to electrical cardioversion. Ibutilide has a conversion rate of up to 75% to 80% in recent-onset atrial fibrillation and flutter; the conversi...

Successful intracardiac electrical conversion of atrial flutter in patients with complex congenital heart disease.

Sixteen patients presenting on 21 occasions with atrial flutter in association with complex congenital heart disease were treated by intracardiac stimulation techniques combined with activation mapping. Nineteen episodes of atrial flutter were successfully converted to sinus rhythm. In the remaining two episodes atrial fibrillation was induced with spontaneous conversion to sinus rhythm within ...

Mapping the conversion of atrial flutter to atrial fibrillation and atrial fibrillation to atrial flutter. Insights into mechanisms.

It is not generally believed that there is a relation between atrial flutter, thought to be due to a single reentrant circuit, and atrial fibrillation, thought to be due to simultaneously circulating multiple-reentrant wave fronts. However, there are many reasons to suggest that these rhythms are more closely related than previously thought. To test the hypothesis that the length of an area of ...

Efficacy, safety, and determinants of conversion of atrial fibrillation and flutter with oral amiodarone.

Amiodarone is effective for long-term maintenance of sinus rhythm after electrical cardioversion of refractory atrial fibrillation or flutter. To examine its efficacy and safety for pharmacologic conversion of these arrhythmias, we studied 129 patients with refractory atrial fibrillation or flutter who had failed previous intensive conventional antiarrhythmic treatment. In anticipation of elect...

Haemodynamic studies before and after electrical conversion of atrial fibrillation and flutter to sinus rhythm.

The clinical use of direct current shock in the management of cardiac arrhythmias (Lown, Amarasingham, and Neuman, 1962) has been amply confirmed by many other workers (O'Brien, Resnekov, and McDonald, 1964; Oram et al., 1964; Morris et al., 1964b). The length of time that sinus rhythm persists after the electrical conversion of chronic arrhythmias is often disappointingly short, even when quin...
